Wyre Integrated Network PCN are recruiting for a Advanced Practitioner to join their team. The successful candidate will join a team consisting of Pharmacists, Paramedics, Social prescribers, Care coordinators, GP Assistants and a Physiotherapist supported by a team of GPs and clinical leads all striving to support the needs of the local community.
Main duties of the jobThe PCN Advanced Practitioner will work autonomously within the community at an advanced level of practice, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting in the community or general practice setting.
The successful candidate will be a vital part of our MDT, and will be expect to provide expert advice and collaborate with a range of clinicians to produce the best healthcare outcomes for our patients.

WIN PCN covers 30,900 patients over four practices in the Thornton-Poulton area and uses a network of skilled clinicians to forge relationships with varying healthcare providers and promote collaborative working through the 4 partner practices, providing personalised care for patients through every stage of life.

Responsible for developing, with patients, a continuing management plan for the health of the patient and when appropriate to refer to other clinicians and agencies,
The successful candidate will work within the multidisciplinary team to deliver the requirements of the QOF, DES and IIF framework as part of the primary care network, whilst also supporting the network.
The role will involve independent prescribing within the scope of professional practice.
Person Specification Experience- Experience of practice within the four pillars.
- Job plan that demonstrates advanced nursing practice and has equity with peers working at this level.
- Experience of prescribing and undertaking medication reviews.
- Experience of working in a primary care environment.
- Experience of working as a practice nurse or community nurse.
- Wound care/removal of sutures and staples.
- ECGs
- Venepuncture
- New patient medicals
- Chaperone procedure
- Requesting and processing pathology tests
- Advising patients accordingly
- Diabetes
- Asthma
- CHD
- Immunisations (routine and childhood)
- Understand the importance of evidence-based practice
- Broad knowledge of clinical governance
- Ability to record accurate clinical notes
- Ability to work within own scope of practice and understand when to refer to GPs
- Knowledge of health promotion strategies
- Understand the requirement for PGDs and associated policy
- Polite and confident, flexible and cooperative
- Motivated, forward thinker
- Problem solver with the ability to process information accurately and effectively, interpreting data as required
- High levels of integrity and loyalty
- Sensitive and empathetic in distressing situations as well as ability to work under pressure/in stressful situations
- Knowledge of public health issues in the local area

- Registered Nurse with Nursing and Midwifery Council.
- Masters degree required for qualification post December 2020.
- Post graduate diploma or degree for Advanced Practice Qualification up to December 2020.
- Qualified Independent Nurse Prescriber on the NMC register.
- Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
- Meets the standards for registered ANP working at advanced level.
- Qualified triage nurse.
- Teaching qualification.
- ALS and PALS.
- Occupational health clearance
- Meet the requirements and produce evidence for nurse revalidation
- Evidence of continuing professional development (CPD) commensurate with the role of an ANP
- Access to own transport and ability to travel across locality on a regular basis
- Flexibility to work outside core office hours
