Senior Manager, Care Management - Transitions of Care

For our client, we are seeking a Senior Manager, Care Management - Transitions of Care to join the team of a leader in the Hospitals & Medical Centers space.

This leader will oversee a high-performing care management function focused on transitions of care, with an emphasis on improving patient outcomes and reducing readmissions. In this role, you will guide a collaborative team of nurses and care coordinators while partnering closely with clinical and operational leaders to strengthen care delivery. The position blends people leadership, quality oversight, and hands-on clinical engagement in a virtual care setting.

Your work will directly influence team performance, interdisciplinary coordination, and the effectiveness of post-acute transition support across the organization.

Location:

Remote

Compensation: $110,000 – $173,000 annually

• Cultivate and inspire a collaborative, patient-oriented team of nurses and care coordinators
• Drive hiring, training, and performance management for frontline teams
• Track staff performance against productivity and quality KPIs to reduce readmissions
• Provide frontline patient care 10% to 20% of the week to understand program dynamics
• Collaborate with clinical and operational leads to optimize patient outcomes

• 5+ years leading high-performing clinical teams in transitional care or care management, preferably in a virtual setting
• 3+ years coaching and developing managers
• Active RN license in good standing
• BSN or equivalent required; master's preferred
• Experience in post-acute medicine, nursing, case management, or geriatrics
• Strong background in interdisciplinary care and change management
