Join our team as an Advanced Nurse Practitioner.

Work within our practices in the Northamptonshire Rural East & South (NRES) Neighbourhood to deliver high-quality, same-day care to our patients.

We're looking for an experienced

Advanced Nurse Practitioner to join our Neighbourhood Access Service, part ofthe innovative New Models Programme. You'll work autonomously and as part of amultidisciplinary team within primary care to provide expert assessment,diagnosis, treatment, and evaluation for patients presenting with minor illness, acute conditions, and common general practice presentations, including women's health.

Main duties of the job

As a Nurse Practitioner you will provide autonomous care for our patients presenting with minor/acute illness,and common general practice presentations including those relating to women'shealth. You will demonstrate safe clinical decision making and provide expertcare by undertaking initial history taking, clinical assessment, diagnosis,treatment and evaluation of care.

Working with GPs and nursing team in the Practice you will continually review and evaluate services and workwith the management team in implementation of change.

About us

The Northamptonshire Rural East& South (NRES) Neighbourhood, consisting of 7 GP Practices serving 88,938patients, is developing an innovative new model of care as part of the NHS New Models Programme.

You will be working with a teamof GPs and specialist ANPs to deliver same day care to the Rural East &South population across our three hub sites.

Job responsibilities

Clinical

Provide clinical care to our patients. This will include reducing the demand on GP time by the assessment and management of patients bymeans of telephone triage and clinic appointments.

Use advanced skills and expert knowledge to assess the physical and psycho social needs of patients with common primary ca reconditions and minor illness, instigating and providing therapeutic treatments based on best available evidence to improve health outcomes.

Use advanced skills and expert knowledge to make comprehensive and focussed assessments, independently and proactively managing care of the patients.

Promote patient independence and self-care where appropriate.

Undertake the role of independent non-medical prescriberin the treatment and care of patients

Review pathology results to ensure appropriate treatment and care

Support and lead the practice for the delivery of the Quality and Outcomes Framework and NHS England/ICB/Federation Contracts.

Regulatory

Respond to complaints and clinical incidents/significant events as appropriate to ensure robust systems are in place for improving practice through joint learning.

Undertake at least one clinical audit a year. Evaluate data and present findings to the Practice Clinical team. Implement changes where appropriate.
