• Lead on post-hospital discharge pathways, ensuring safe and effective transitions for adults requiring ongoing care via the Home First initiative or step-down beds.
• Oversee and contribute to system calls, sharing updates, insights, and actions to enhance service efficiency and client outcomes.
• Supervise and support social workers and unqualified staff, maintaining high-quality standards of practice and professional development across the team.
• Conduct Care Act assessments and demonstrate thorough knowledge of the Mental Capacity Act and Safeguarding principles, ensuring all practice is compliant and person-centred.
