Father Bill’s & Main Spring seeks a Triage Outreach Worker in Plymouth to conduct street outreach, assess needs, and coordinate services toward stable housing. The role requires a Bachelor’s degree or equivalent experience, with a strong focus on outreach to unsheltered individuals and data-driven case management.
You will build partnerships with internal teams and external providers while maintaining confidentiality and advocating for clients' housing outcomes.
