Burlington Case Manager Position JACC

Provides case management for a community-based program along with service and resource coordination in a human services environment with clients from multiple socioeconomical statuses.

Primary

Job Responsibilities & Duties

• Conducts phone intake assessments in Burlington County, New Jersey.
• Encourages clients and/or their families to facilitate achieving goals, developing life skills, and/or sustaining stability in the community, to promote self-determination and overall wellbeing
• Facilities connections and referrals to resources in the community.
• Attends community meetings and/or outreach events to promote connections within the community with staff members.

Job Duties

• Advocates on behalf of clients and families for services, basic needs, and other related issues.
• Composes or prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications (Microsoft Word, Outlook, and/or Excel).
• Stay well-informed of community resources and procedures for referring clients to appropriate resources.
• Performs other duties and assumes other responsibilities as assigned by manager/director.
• Works with community partners to maximize the assistance for clients.

Supervises

This position may require employee to supervise BSW Interns.

Education & Experience

• Required Bachelors' degree in Social Work (BSW) and/or Degree in Behavioral Studies-Human Services/Social Services with major course work in social work, psychology or another field closely related field.
• At least one (1) year of experience in social case work, counseling, eligibility determination, social service or community resource referral, or closely related work.
• Ability to communicate effectively with a diverse audience.
• Knowledge of general interviewing/intake process.

Pay

$16.00 – $17.00 per hour
