Health Care Assistant, Long Term Care/Rehabilitation - Queens Park Care Centre - Casual Posting at Fraser Health Authority — Casual posting in New Westminster. Join our compassionate and collaborative team as you work under the direction of nurses and with the interprofessional care team to assist residents with daily activities, document their progress, and ensure their comfort and well-being.

Queens Park Care Centre provides rehabilitation services, an adult day program and a long-term care community. It has an outpatient rehab service and 76 specialized inpatient rehabilitation beds, serving populations including Brain Injury, Spinal Cord Injury, Stroke rehab, lower leg amputees, complex neurological conditions and other musculoskeletal injuries.

Responsibilities
  • Following an established, individualized care plan, provide person-centered care by assisting residents/clients in activities of daily living. Encourage and assist with toileting, bathing, skin care, grooming, dressing, personal laundry and mobilizing residents/clients.
  • Participate, as a member of and in collaboration with the interdisciplinary team, in the organization and implementation of established leisure activities as outlined in the care plan; encourage resident/client participation and inform residents/clients, families and volunteers of daily plans.
  • Demonstrate actions and movements to residents/clients and encourage the involvement of family members and volunteers in specific, appropriate 1:1 activities per the care plan.
  • Observe changes in client behavior, abilities, cognitive skills and program participation; document and report changes to the interdisciplinary team as appropriate.
  • Perform procedures such as taking temperature and weight as directed; obtain laboratory specimens as needed.
  • Utilize mechanical aids (lifts, wheelchairs and stretchers) to transfer and position residents/clients in comfortable and therapeutic body positions.
  • Perform limited food preparation (e.g., making tea, coffee, toast) and assist residents/clients with dining as required.
  • Welcome new residents/clients and their families by orienting them to the environment, routines, staff, volunteers and other residents/clients.
  • Participate in interdisciplinary team meetings, in-services, team and family conferences and other meetings/committees as assigned.
  • Provide input on the effectiveness of programs and routines and suggest changes or new programs to the interdisciplinary team.
  • Clean and maintain equipment as needed; report unsafe or malfunctioning equipment, hazardous conditions, maintenance needs and supply needs to appropriate staff.
  • Perform other related duties as assigned.
Qualifications

Education, Training & Experience
Grade 10; graduation from a recognized Health Care Assistant Program or an equivalent combination of education, training and experience.

  • Ability to organize work.
  • Ability to operate related equipment.
