- Individualized pharmacotherapy – tailoring medication therapy based on chart review, laboratory data, and patient assessment.
- Patient medication counselling – reviewing literature, summarizing information, and demonstrating procedures and equipment to patients and families.
- Work in collaboration with physicians, nurses, and other health‑care providers to develop, implement, and monitor therapeutic plans.
- Assist in developing drug‑therapy guidelines for individual disease conditions and monitor their effectiveness.
- Participate in medication‑use evaluations, quality‑improvement initiatives, and clinical drug trials.
- Act as a department resource person, providing detailed drug information on issues related to drug therapy.
- Contribute items such as case studies, review articles, and other content for professional journals.
- Provide instruction and/or supervision to pharmacy students and residents.
- Perform drug‑distribution duties, including interpreting prescriptions, dispensing medications, and documenting all activities in the patient health‑care record.
- Participate in quality‑improvement and medication‑safety initiatives and document outcomes.
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Completion of an accredited Hospital Pharmacy Residency Program or equivalent combination of education, training, and experience.
- Broad knowledge of organization policies, evidence‑based clinical pharmacy practice, medication‑use evaluation, and research methodology.
- Ability to apply clinical pharmacy knowledge and make decisions with minimal supervision.
- Strong verbal and written communication skills and the ability to establish effective working relationships.

SalaryThe salary range for this position is CAD $51.37 – $64.19/hour.
