Position Summary

The RN/LPN, Surgical Services – Preadmission Clinic

at False Creek Healthcare Centre performs comprehensive preadmission assessments to ensure patients are ready for scheduled surgical procedures. The RN interviews patients, reviews medical histories, identifies potential risks, coordinates and evaluates diagnostic testing, and communicates findings to Physicians and Surgeons.

The RN/LPN provides patient and family education on disease management, risk reduction, medications, and health maintenance. Working collaboratively with the interprofessional care team, the RN develops and updates individualized care plans, documents assessments accurately, and contributes to quality improvement, clinical education, and mentoring initiatives within the clinic.

Key Responsibilities

  • Perform preadmission screening of surgical patients through interviews, health history reviews, and assessment of potential risks.
  • Coordinate required diagnostic tests, review results, identify abnormalities, and report findings to Physicians/Surgeons.
  • Conduct comprehensive nursing assessments, develop nursing diagnoses, and plan, perform, and evaluate nursing interventions.
  • Receive and prepare patients for testing, ensuring comfort and understanding of procedures; perform non-invasive diagnostic tests as required.
  • Document assessments, interventions, outcomes, and referrals accurately according to clinical standards.
  • Assess learning needs of patients and families; provide education on risk reduction, medications, lifestyle adjustments, and health maintenance.
  • Collaborate with patients, families, and the interprofessional care team to develop, implement, and update individualized care plans.
  • Participate in discharge planning, referral processes, and coordination with community resources.
  • Contribute to quality improvement, research, and clinical program evaluation initiatives.
  • Act as a preceptor or mentor for new staff and students; participate in professional development and continuing education programs.
  • Participate in nursing committees, care conferences, and councils, providing feedback on policies, procedures, and clinical tools.
  • Perform other related duties as assigned.

Requirements

  • Current registration in good standing with the British Columbia College of Nurses and Midwives (BCCNM).
  • Completion of an accredited nursing education program combined with 2-3 years recent experience
  • Minimum 2 years of recent relevant clinical experience in surgical services, preadmission, or related acute care areas is an assest
  • Completion of post-graduate courses or certifications relevant to surgical services preferred.
  • Clinical proficiency in patient assessment, diagnostic reasoning, and nursing interventions.
  • Strong communication, collaboration, and patient advocacy skills.
  • Competence in operating relevant medical and diagnostic equipment and electronic documentation systems.
  • Ability to mentor, precept, and educate patients, families, and staff effectively.
  • Physical ability to perform required duties.
