Mount Farm Surgery has a fantastic opportunity for an experienced nurse to join our team on a part-time basis with hours and days negotiable.

Main duties of the job
  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long-term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management.
  • Running asthma, COPD and diabetes clinics and reviews.
  • New patient and health checks.

Mount Farm Surgery is a modern, efficient, and well-equipped GP surgery in the Moreton Hall area of Bury St. Edmunds. First established in Looms Lane in Bury St. Edmunds, we moved to the Moreton Hall Estate approximately thirty years ago, which was built on the original Moreton Hall farmland at the top of Mount Road (hence the name Mount Farm).

Proactive and holistic care

We endeavour to offer proactive care to ensure the best of health for our patients. Often health issues are related to other aspects of a patients lifestyle, and we believe in helping patients improve their overall health and well-being, wherever possible taking the opportunity to prevent illness and not just treat it. We routinely check up on patients with long-term conditions to ensure we are providing effective care.

We also run a series of clinics and patient workshops that provide valuable help and support to those dealing with long-term conditions such as asthma and diabetes.

A GP training practice

Some of our GPs have the additional skills, experience, and qualifications to support qualified registrars through their vocational training to become a GP.

Mount Farm Surgery is a member practice of the Suffolk & North East Essex ICB, which is committed to the ongoing improvement of local services through working with West Suffolk Hospital, community services in Suffolk and the Suffolk Mental Health Partnership Trust.

Qualifications
  • Current NMC registration with an active PIN.
  • Ideally having completed the CNO
    002 Discovering a Nursing Career in General Practice module or the Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) however we would still be interested in those applicants with equivalent experience or qualifications.
  • Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
Experience
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
