Your role and responsibilities:

We are looking for a passionate practitioner with a positive mindset to join the Flexible Care and Support Team in the West of Suffolk.

Your role will see you work alongside colleagues both within the organisational and outside, including health colleagues, allocations lead, Business, Finance and Service Development and Contracts teams and wider partners, putting you in an ideal position to develop, build and help your experience in Adult Social Care.

The main purpose of this role is to work with adults to identify and deliver good quality outcomes to support and maximise independence and wellbeing.

You will be working as part of a team to promote personalisation, independence, wellbeing and choice to adults and to carry out asset-based assessments /reviews.

You will need:

You will need experience of creating reablement plans in line with a sign of safety solution focused approach and have an understanding of The Care Act 2014, albeit training will be provided.

The team:
  • West Flexible Care and Support Team are responsible for coordinating and allocating adult safeguarding enquiries.
  • Furthermore, the team are responsible for attending Continuing Healthcare Meetings on behalf of Customers who experience complex health needs.
  • You will be expected to have an allocated caseload reflecting customers requiring Care Act Assessments of need and you will be expected to undertake assessments of carers too.
  • It is essential that you have excellent interpersonal skills, empathy and consider equality and diversity of the customers we support as a team.
