Job Overview

An exciting and rewarding opportunity has arisen for an Administrator to join the team in the West Integrated Transfer of Care Hub (iToCH). Currently based in Camborne Health Office, the role will involve hot‑desk and shadow staff during induction at Helston Community Hospital and Penwith Health Centre. The Administrator will support clinical staff with admission avoidance, arranging community support, case managing non‑urgent referrals and processing referrals.

The role requires cross‑boundary and collaborative working with multidisciplinary teams, Adult Social Care and external agencies.

Main Duties
  • Monitor and review STRATA referrals to identify and process work for community health and social care teams.
  • Manage a varied workload in a busy working environment – organise, prioritise and respond to enquiries.
  • Assess risk and priority of routine referrals and decide referral pathways.
  • Review key blockages to patient flow, ensuring patients achieve independent outcomes and highlight issues to Clinical Prioritise rs and managers.
  • Maintain mailbox, phones and Electronic Referral system.
  • Gather information using clinical and professional systems to process referrals.
  • Escalate non‑routine pathway referrals promptly and accurately.
  • Document decisions on STRATA, RiO and Mosaic in line with organisational policies.
  • Maintain confidentiality and comply with statutory responsibilities.
  • Use organisational resources efficiently to minimise waste and cost.
Qualifications
  • Good standard of education, including GCSE English & Maths or equivalent.
  • NVQ Level 3 in Administration or equivalent.
  • Experience of working within a community setting.
Essential Skills and Aptitude
  • Previous experience in a healthcare office environment.
  • Computer literate with excellent knowledge of Microsoft Office, Excel and PowerPoint.
  • Excellent verbal and written communication skills.
