Our client, a well-established social care organization based in South - Fawcett House, is currently seeking a dedicated and experienced Social Worker - Assessment Team to join their team on a contract basis.

This role offers a competitive pay rate of £39 per hour umbrella and provides a hybrid working environment.

Candidate Responsibilities:

Conduct assessments to determine the needs of individuals and families requiring social care support
Develop and implement care plans tailored to the specific needs of each service user
Work collaboratively with other professionals, such as healthcare providers and community services, to ensure comprehensive support
Provide advice and guidance to service users and their families on available support services
Maintain accurate and up-to-date case notes and records in accordance with data protection regulations

Required Skills:
