You are a committed individual with a caring nature, capable of adapting to the dynamic and fast-paced environment of The Community Resource Team Homecare in Cardiff. Fluency in languages, particularly Welsh and other community languages, is a plus. You are willing to train for the necessary qualifications and are focused on safeguarding children and vulnerable adults.

You will support clients in achieving their reablement goals, working in collaboration with health professionals. Your duties will be guided by a service delivery plan and allocated via a mobile device. The role involves working shifts according to a rota, including potential evening and weekend work. You will help facilitate independent living for service users by preventing unnecessary hospital admissions and assisting with hospital discharges.

• Good observation and recording skills
• Flexible and focused approach
• Ability to train for necessary qualifications (HSc)
