Key Responsibilities
  • Conduct comprehensive assessments of children, young people, and families in line with statutory frameworks and Cardiff Council procedures.
  • Develop, implement, and review personalised care and support plans to meet individual needs.
  • Provide guidance and support to families, carers, and professionals to enable effective care delivery.
  • Collaborate with multi-agency partners, including health, education, and social care, to ensure integrated services.
  • Safeguard the welfare of children and young people, promoting their safety and wellbeing at every stage.
  • Maintain detailed and accurate records compliant with Council standards and legal requirements.
  • Participate in professional supervision, team meetings, and ongoing professional development.
