Nurse Behavioral Health Field Care Manager

About This Role

The Nurse Behavioral Health Field Care Manager assesses and evaluates members' needs to achieve optimal wellness by guiding them toward appropriate resources. This role is ideal for experienced nurses who are skilled in case management and have a passion for supporting members' health.

Highlights

• Pay: $71,100 - $97,800/year
• Location: Remote, SC
• Department/Unit: Care Management
• Schedule: Full-time, Monday through Friday, 8:00 AM - 5:00 PM Eastern Time
• Residency: Must reside in the State of South Carolina
• Travel: 50% travel required

What You'll Do

• Assess and evaluate members' needs to maintain optimal wellness.
• Guide members and families toward appropriate resources.
• Employ strategies to manage members' physical, environmental, and psycho-social health issues.
• Identify and resolve barriers to effective care.
• Monitor patient care through continuous assessments and evaluations.
• Create member care plans as needed.
• Collaborate with staff, providers, members, and families through face-to-face meetings.

Requirements

Education:

• Bachelor’s in nursing (BSN) required

Experience:

• 2+ years of experience in case/care management
• Preferred: 3+ years of in-home assessment and care coordination experience

Licenses &

Certifications:

• Active RN license in the state of South Carolina required
• Certification in Case Management preferred
• Motivational Interviewing Certification or knowledge preferred
