** In this community nursing role, as a Registered Nurse (RN) you will have scheduled visits to care for patients with palliative care needs as part of a palliative care team comprising of Nurses, PSW’s and Allied Health members. You will work with patients of all ages with differing complexity of care and play a key role in helping patients navigate through their care journey.

* What you’ll do:

*** Using appropriate assessment tools/techniques to assess patients’ individualized needs
* Executing and continuously monitor care plans
* Engaging patients using a collaborative approach by building rapport and trust
* Maintaining and update professional patient records related to patient encounters and plans
* Liaise with other members of the client’s palliative care team
*
* What you bring:

*** Registration with the CNO as a Registered Nurse in good standing
* Palliative care and PPS trained:
Palliative Care Fundamentals / CAPCE and LEAP Core Training (or will be trained within 3 months upon hire) preferred.
* Valid driver’s license and access to a vehicle is required
* Willingness to travel to and from patient residences within a designated area
* Passion for making a difference in your community
* Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs##
