Community Physiotherapist

We want you:If you have a passion for rehabilitation, thrive working within multidisciplinary teams, and have innovative ideas to develop and deliver personalised care plans that support people to maintain their health and wellbeing, and promote hospital admission avoidance. You will need excellent communication skills and commitment to high quality patient care alongside flexibility and resilience to meet the challenges of community working.

As well as work within core physiotherapy, opportunities exist to work alongside referral centre (urgent response) and Home First therapists, so there's plenty of scope for your professional development.

We will give you:A chance to work across adult integrated health and social care services, supported by Clinical specialists, Advanced Practitioners and Professional Leads, to help you grow and develop your professional and clinical skills in a variety of areas, so that you can achieve competence and specialisms in your particular field of interest.

As a Band 5 you will need:

• Relevant experience of working as part of an integrated multi-disciplinary team
• Excellent communication and IT skills
• Experience of working in people's homes
• Be highly motivated to provide evidence based practice
