ROLE PURPOSEThis position will provide outreach support to clients referred to Connective by our funders. You will work to enable clients’ adjustment to and success with life in the community.
LOCATIONLocation for this role is primarily at the Provincial Assessment Centre (PAC) in Coquitlam, with training conducted on‑site at PAC and the Tri‑cities Office. The regular work site will be the Provincial Assessment Centre.
STATUS AND CLASSIFICATIONUnionized. This position is open to applicants of all genders.
HOURS (20 HOURS PER WEEK - TEMPORARY)Wednesday to Sunday, (1300 - 1700)
Please note this a temporary position until March 2026.

- Participates as a member of Connective team to ensure quality and continuity of services to clients
- Liaises with staff and volunteers of community and government agencies as required
- Liaises with Connective staff as necessary
- Accepts clinical direction from the care team

ACCOUNTABILITIES
- Receives referrals from funders
- Works with the client, researches background material, and seeks the input of other professionals and family members, to provide the assessment that will ascertain needs and postulate intervention possibilities
- Works with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long‑term goals
- Ensures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment, and education are addressed in this plan
- Supports client in various environments, which may include exposure to bedbugs, animals, needles, blood, lice, human waste, hording and other
- Ensures the client’s ongoing legal, mental, and physical health issues are addressed in the plan
- When and as needed, provides a wide range of interventions to enable the client to address and accomplish the established goals
- Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long‑term goals
- On the client’s behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric services
- Maintains extensive records and case notes in accord with Community Living British Columbia policies and standards
- Completes monthly progress reports
- Participates in staff and resident/staff meetings as instructed by the Program Manager
- Other duties as subsequently negotiated
- Functions independently, while remaining a crucial member of the care team
- Adheres to Connective policies and procedures
- Is available to work flexible hours as needed, which may include some evenings/weeksends, as determined by program/client needs and schedules
