Job SummaryThe Primary Care Pharmacy Technician will work alongside the practice Clinical Pharmacist to support the clinical team in all aspects of prescribing and medicines safety. The role aims to maximise safe prescribing and improve patient care, acting as a point of contact for patient prescription enquiries, conducting medicines reconciliation after discharge and other transfers of care, and providing assistance with high‑risk drug monitoring.

Job Description Responsibilities- Support the clinical pharmacist and team to provide safe repeat prescribing service.
- Comply with legal and professional requirements and safe systems of work for re‑authorising repeat prescriptions.
- Coordinate medication review appointments and medication changes advised by the clinical team.
- Medicines reconciliation post‑discharge and on other transfers of care.
- Day‑to‑day handling of prescription queries within own competence.
- Use and maintain incident reporting, prescribing data and patient outcomes databases.
- Participate in practice and other meetings to improve prescribing issues.
- Plan and organise own workload to meet deadlines.
- Support Share Care monitoring and processes.
- Deliver training and development to practice based prescribing clerks and administrators.
- Escalate problems or issues to line manager as needed.
- Maintain effective working relationships with GP practices, pharmacy staff and other healthcare professionals.
- Liaise with pharmacies on drug supply issues and recommend alternatives.
- Document and coordinate sensitive information in accordance with GDPR, Caldecott guidelines and confidentiality code.
- Conduct prescribing audits using GP practice clinical systems.
- Assist in UHB Medicines Management Incentive Scheme activities.
- Ensure timely action on MHRA drug alerts.
- Promote equality, diversity and openness in all practices.
- Support safeguarding and protection policies.
- Experience of working in primary care.
- GCSEs A–C (or equivalent).
- Knowledge of prescribing practices and medication safety.
- NVQ Level 3 in Pharmaceutical Science.
- Experience with repeat prescribing systems.
Rehabilitation of Offenders Act (Exceptions) – a DBS check is required.
