We are working with a client in Crewe who is seeking a Social Care Assessor to support vulnerable adults and their careers. The role involves completing needs-based assessments, coordinating care packages, and managing a caseload in line with legislation and local policies.
Key Responsibilities
- Complete needs-based assessments and develop individual Care Plans
- Manage and review care packages, responding to changing needs
- Act as Care Manager, coordinating services and support
- Liaise with families, professionals, and external agencies
- Maintain accurate and up-to-date case records
- Ensure safeguarding and risk management procedures are followed
Key Requirements
- NVQ Level 3 in Social Care (or equivalent)
- Experience working with vulnerable adults in a social care or health setting
- Knowledge of safeguarding, risk assessment, and care planning
- Ability to work collaboratively with multi-disciplinary teams
- Good communication, organisation, and IT skills
- Flexible approach and ability to travel within the borough
Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: 15- 20 per hour
Working pattern - 37 hours per week.
