Home Rise is hiring a full-time Case Manager to provide case management services to residents in scattered-site permanent supportive housing on Treasure Island, San Francisco. You will support families facing homelessness with housing retention, referrals, and crisis interventions, guided by harm reduction and Housing First principles.
The role requires a high school diploma (BA/BS preferred), bilingual Spanish preferred, and experience in social services.
