Job Summary
The post holder is a Nurse Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi‑disciplinary team within general practice. The post holder will contribute to providing nursing care to patients registered in the GP practice. The post holder will focus on assuming a lead role in specified areas, e.g., vocational and employment advice to patients, activities coordination, assisting in individual and therapeutic group work.
Key Responsibilities- Engage in positive relationships with patients, carers, families and other staff based on respect and within appropriate boundaries.
- Act in a professional capacity at all times.
- Communicate with the patient about care planning, risk assessment and needs.
- Communicate complex and sensitive information verbally and in writing.
- Actively contribute during handovers to meet the care needs of patients and in staff meetings.
- Convene patient information in the role of an associate nurse for MDT meetings and articulate an understanding of MDT structures and member roles.
- Formulate care plans under the supervision of registered staff.
- Communicate risk issues as they arise to ensure safe care, and communicate concerns relating to patient behaviour or mental state to other members of the multidisciplinary team.
- Undertake routine clinical procedures including blood pressure monitoring, venepuncture, ECG recording, wound care and dressings, injections and immunisations (where trained and competent), health checks and long‑term condition monitoring.
- Support chronic disease management clinics (e.g., diabetes, asthma, COPD) under supervision.
- Recognise and respond appropriately to changes in a patient’s condition, escalating concerns to a registered clinician.
- Maintain accurate and timely patient records using the clinical system (Systm One).
- Registered Nursing Associate with the Nursing and Midwifery Council (NMC)
- Experience of working in a healthcare setting
- Ability to work within scope of practice and recognise own limitations
- Good communication and interpersonal skills
- Basic IT skills and ability to use clinical systems
- Commitment to patient‑centred care and confidentiality
- Willingness to train to complete Cervical Screening and Childhood Immunisations
- Demonstrated knowledge of adult physical health problems (e.g., high blood pressure, diabetes, obesity, epilepsy, respiratory issues, constipation)
- Ability to complete ongoing monitoring templates and health assessments with team support
- Understanding of verbal and non‑verbal communication skills at the desired level
- Knowledge of risk behaviours and risk management, and incident reporting
- Experience working in primary care or a GP practice
- Experience with chronic disease monitoring and knowledge of QOF and primary care pathways
- Phlebotomy and vaccination experience
- Planning and organisational skills, and ability to plan and manage workload
- Capability to facilitate/co‑facilitate group activities focused on health promotion or social activity
Salary: £13 to £14 an hour (DOE)
