Substance Misuse Case Manager - Outreach Team
Aspire is a partnership between ADS and Rotherham, Doncaster and South Humber NHS Foundation Trust (RDaSH). It combines the strengths of both NHS provision under the leadership of a consultant with the strengths of the voluntary sector provided by ADS. The partnership has worked together for over 20 years providing Drug and Alcohol Services to our local communities. It is a forward thinking and innovative partnership that works closely with the recovery communities it serves to provide flexible, responsive services which offer the best possible outcomes for its service users and the community it serves.
It is a very exciting time to be working in the drug and alcohol field, with significant new career opportunities being created.
The outreach team will engage with the rough sleepers and the hard-to-reach community, as well as those with physical health needs and support them to enter the treatment system and the wider local community services. The successful candidate will devise a holistic care package, following an assessment of risk and need. You will be required to work flexibly to meet the needs of this group.
If you have relevant experience in the substance misuse field, a relevant degree in psychology, sociology, social work or diploma in Adult Care or equivalent we would like to hear from you.
If you are experienced by working in the field but do not have the formal qualifications, we provide a comprehensive package of accredited qualifications and Social Workers ASYE
Main duties of the jobTo work across various outreach and other settings in Doncaster to engage people with substance misuse problems, providing assessment, motivational therapeutic interventions in group and 121 settings and ensuring access to community services to provide holistic support.

responsibilities
MAIN RESPONSIBILITIES:
1. Principal accountabilities
Engage with potential service users in a variety of outreach settings and signpost to treatment services and other relevant agencies in the locality
Undertake comprehensive assessments to assess the eligibility and suitability, strengths and needs of a service user in relation to the service
Conduct risk/need assessments and implement risk management/care plans in line with policy and procedures.
Deliver care packages including, where appropriate, psychosocial, and psychological interventions that are often time limited.
Liaise with other health and social care professionals where necessary and appropriate
Function as part of a multi-disciplinary team, contributing to team meetings as required.
Case-manage in line with a recovery focused model
To communicate highly sensitive information within an often hostile and emotive environment to service users, relatives, and colleagues
To assist in the decision-making process regarding the referral of service users children to childrens services assessing and interpreting complex and conflicting information
Ensure all possible steps are taken to account for personal safety of staff and service users and elevate areas of concern where appropriate.
Compile reports and participate in case conferences, multi-agency and multi-disciplinary team meetings and any other appropriate care planning meetings in relation to service users.
