Overview
Glu Care Integrated Diabetes Center is seeking a dedicated DHA Registered Nurse to join our clinical team in delivering exceptional patient care. The ideal candidate is compassionate, adaptable, and committed to providing high-quality nursing services in a modern, fast-paced environment. Experience in chronic disease management, patient education, and digital tools is a plus.
Responsibilities- Provide direct patient care, including vital sign checks, blood glucose monitoring, and patient observations.
- Educate patients on diabetes management, including lifestyle tips and monitoring instructions, ensuring compliance with care plans.
- Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards.
- Assist in the preparation and handling of equipment for procedures, wound care, and specimen collection.
- Communicate effectively with patients, providing empathy and clarity to address their questions and concerns.
- Reinforce health education provided by physicians, encouraging self-care practices and adherence to treatment plans.
- Accurately record and update patient information, treatment plans, and observations in electronic medical records (EMR).
- Report any changes in patient condition to the supervising nurse or physician promptly.
- Adhere to all DHA regulations, nursing guidelines, and Glu Care protocols to ensure the safety and well-being of patients and staff.
- Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support.
- Valid DHA Assistant Nurse License – A MUST
- Minimum of 1-2 years of experience in a clinical setting, preferably in endocrinology, diabetes care, or chronic disease management
- Knowledge of diabetes management and patient education in lifestyle-related chronic conditions is preferred
- Strong communication and interpersonal skills with a patient-centric approach
- Basic proficiency in electronic medical records (EMR) systems is preferred
- Compassionate and empathetic demeanor with a commitment to patient care
- Ability to work effectively in a multidisciplinary team environment
- Good organizational and time-management skills
- Strong attention to detail and adherence to clinical protocols
