Humana iCare is seeking an RN Care Manager to join the Family Care Partnership program serving Milwaukee and surrounding counties. You will conduct home and community visits, complete nursing assessments, and ensure members have access to appropriate care and services.
The role reports to the Manager of Care Management Support and requires Wisconsin RN licensure, 2 years of related health care experience, and willingness to travel up to 50% within Milwaukee area.
