What you will do:

  • Perform comprehensive assessments to support discharge planning and provide guidance tailored to frailty, medical, and acute settings.
  • Collaborate with multidisciplinary teams to facilitate seamless patient discharges.
  • Implement and review care plans that align with patient goals and promote independence.
  • Provide expert advice and support to patients and their families to ensure a smooth transition from hospital care to home or another appropriate setting.

To be successful in this role, you must have HCPC registration and possess experience working within acute medical environments or care of the elderly settings. A valid driver’s licence is also necessary as you’ll need to travel within the community.
