The post holder will be the clinical advisory lead within the Improving Immunisation Uptake Project Team set up to work with GP Practices, PCN's and the ICB for the delivery of the commissioned programme which aims to improve immunisation offer and uptake across Kent and Medway within GP Practices, with a particular focus on data transfer, GP Practice Processes, GP Practice Staff awareness of programmes and procedures and reducing health inequality.
The objective of the Improving Immunisation Uptake (IIU) initiative is to utilise Child Health Information (CHIS) General Practice (GP) level data and clinical advisory input to improve vaccination uptake in 0-5-year olds, assessing and identifying areas of improvement within GP Practice's which could involve a particular focus on specific groups of children and communities where vaccine uptake is lower than expected.

Main duties of the jobBand 7 colleagues are overall responsible for the management, coordination and implementation of the work, including the detailed planning, effective coordination and management of a range of complex activities and related business and administrative tasks. Band 7 colleagues are required to develop detailed reports and other related documentation based on analysis and interpretation of a range of often complex information, tracking and managing actions.
This is achieved by designing, developing, implementing and maintaining systems that work to fully support the patient or service user journey and eliminate wastage and duplication with a business model emphasis.
Including:
- Assessing, planning, implementing and evaluating holistic care in relation to care planning/pathways
- Developing integrated care programmes in partnership with service users, commissioners and key stakeholders.
- Actively working as a member of the integrated team to ensure high quality care is provided to service users.
- Having the ability to synthesise information in order to present proposals effectively to decision makers (including Directors and Senior Executives) in ways which facilitate their understanding and influence pathways
- Making recommendations to decision makers in order to drive forward innovative solutions to achieve business objectives, service improvement strategies and financial efficiencies linked to packages of prescribed care or identified projects
