Primary Care Licensed Practical Nurse

Deliver primary care nursing at Fort Smith Primary Care Clinic, using patient assessment and clinical support skills within an interdisciplinary team.

The Northwest Territories Health and Social Services Authority seeks a Licensed Practical Nurse for its Fort Smith clinic. You will provide direct nursing care, health education, and wellness support for individuals, families, and communities. The position emphasizes patient-centred and culturally safe care.

• Provide direct patient care in primary care settings
• Perform assessments, vital signs, treatments, and nursing procedures
• Assist physicians and nurse practitioners with examinations
• Coordinate referrals, follow-up care, and Telehealth appointments
• Maintain documentation and protect patient confidentiality

• Complete a Licensed Practical Nursing Program
• Complete Medication Administration and relevant continuing education
• Have two years of interdisciplinary model experience
• Be eligible for Northwest Territories and Nunavut registration
• Provide a satisfactory vulnerable sector check
