Elderplan is seeking a Registered Nurse in New York State to support Medicare and Medicaid managed care members. The role requires CPR and at least 1 year of acute or Medical-Surgical nursing experience.
You will advocate for patients during care transitions and collaborate with discharge planners and case managers by phone. Strong critical thinking, customer service, and problem-solving skills are essential to provide quality, person-centered care within Elderplan's SNP program.
