We are looking to recruit an enthusiastic Clinical Lead to join Princeway Community Care Team in the Frodsham area. You will be joining a multidisciplinary team including but not limited to Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses, AHPs as well as many other specialist professionals who visit the team.

We are looking for someone who is enthusiastic, self‑motivated and who provides confident support and clinical leadership to clinicians alongside working in collaboration with other professionals. Has excellent communication skills and an ability to work with patients and their families/carers and when necessary, act to safeguard individuals who are at risk from harm and abuse.

This post is part time 22.5 hours per week, the service runs between 8am – 8pm, 7 days a week. There will be an expectation that you will participate on the rota until 8pm.

Key Responsibilities
  • Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload
  • Working collaboratively with the wider health, social care and the third sector partner agencies to meet the needs of patients
  • Ensure patients receive the right care, by the right person at the right time and the right place
  • Placing the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, with minimal avoidable delays, and an emphasis on people achieving independence whilst improving quality of care
  • To provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives
  • To be aware of local public health community profiles and how this influences the required care for the population
  • According to community profiles: understand the skills required from the Community Care Team
  • To focus on the delivery of person centred care
  • To provide effective, evidence based care to a defined population through case load management and monitoring of care standards
  • Provide clinical leadership
