Licensed Practical Nurse – Geriatric Care

Full Time Evenings, Every Other Weekend

Assignment Details

1 year experience as LPN is required.

Requirements

• Current NY LPN Licensure
• BLS certification (AHA or RQI required)
• Knowledge and skills to provide care appropriate to the age of the patients served
• Ability to assess and interpret information relative to age-specific needs
• Ability to provide care to geriatric residents

Responsibilities

• Collect ongoing data relevant to resident's physical and psychosocial condition and communicate findings with the RN and interdisciplinary team.
• Contribute to the development of an age-appropriate and culturally relevant care plan.
• Recommend revisions to the care plan after evaluating resident outcomes and collaborate with families and team members.
• Maintain current knowledge and competency in nursing practice and participate in staff mentoring/orientation.
• Ensure the delivery of care considers factors related to safety, cost, and impact.
