LPN – Nursing
Full Time, Evenings, Every Other Weekend
1 year experience as LPN required.
Current NY LPN Licensure
BLS certification (AHA or RQI required)
Provide direct and indirect skilled nursing care for assigned residents.
Collect data relevant to resident's physical and psychosocial condition and communicate findings to the RN and other members of the Interdisciplinary Team.
Contribute to the creation of appropriate care plans involving residents and families.
Recommend revisions to care plans based on evaluation of resident outcomes.
Maintain current knowledge and competency in nursing practice.
Participate in mentoring and orientation of staff.
