AboutThe Role
Join our innovative Interface Division, part of NHSGGC’s Transforming Together programme, and play a key role in delivering modern, patient-centred care. As a Registered Nurse within the Flow Navigation Centre Plus (FNC+Plus), you will be at the heart of a service designed to improve patient flow, reduce unnecessary hospital admissions, and ensure timely access to the right care.
What You’ll Do- Provide clinical triage (follow pathways) via telephone and digital platforms.
- Act as a single point of contact for referrers and professionals across hospitals, community teams, out-of-hours services, and social care.
- Use your nursing expertise to assess, prioritise, and direct patients to appropriate care pathways, including Virtual Hospital and Rapid Assessment units.
- Work collaboratively with a multidisciplinary team including consultants, ANPs, and Scottish Ambulance Service colleagues.
- Be part of a pioneering service that uses technology to enhance patient care, not replace it.
- Contribute to reducing pressure on acute services and improving patient experience.
- Access opportunities for professional development and involvement in shaping future models of care.
- Current NMC registration as a Registered Nurse.
- Evidence of continued professional development.
- Strong clinical decision‑making skills to support safe and effective practice.
- Highly developed communication, listening and interpersonal skills.
- Ability to work in a fast‑paced, digitally enabled environment.
- Excellent team‑working skills with the ability to work effectively as part of a multidisciplinary team and use own initiative.
- Excellent time management and organisational abilities.
- Working knowledge of basic information technology systems (with further development provided through in‑house training).
- Experience of unscheduled care.
- Broad range of knowledge across several specialities.
- Familiarity with digital health tools and remote monitoring systems.
