REABLEMENT SUPPORT WORKER

Location: Across the Mendip District, Street, Glastonbury, Shepton Mallet, Frome & Wells.

Salary: £12.60 per hour, plus paid travel time and mileage.

Please note we are not offering Sponsorship for this role.

Join our Reablement Team and become part of the Candlelight Care family.

We require staff to join our D2A (Discharge to Assess) service across the Mendip area, delivering reablement support and completing assessments, with a focus on promoting independence.

D2A works closely with the NHS, working collaboratively with a range of professionals to deliver the best support possible to newly discharged patients.

To be eligible for this role you must have the following:

• A full valid driving license and access to your own vehicle.
• Be available to work on a two-week rolling Rota, working alternate weekends, or weekends only.
• Excellent communication skills- both verbal and written
• Basic IT skills- although full training will be given

Our Current Vacancies

Week 1
- Saturday- 7-2

Sunday- 7-2

Week 2
- Monday- 7-2 and 4-10

Friday- 4-10

Saturday- 4-10

Sunday- 7-2

There is some flexibility with start and end time of shifts.

This role offers contracted hours, your face-to-face patient visits and administrative tasks are allocated within your paid block.

We are particularly keen to recruit weekend staff that can boost our weekend capacity.

Grade 5: £12.60/hr, paid travel time and mileage, uniform, Blue Light Discount Card.

Mendip/Somerset areas

Previous Care Experience Is Desirable But Not Essential.
