The Community Inpatient Unit providesresidential rehabilitation focusing on a multi-disciplinary approach providinga holistic assessment of needs. A bed-based Enhanced Recovery service which provides Rehabilitation andre-ablement opportunities for adults in North East Lincolnshire. The service will promote faster recovery from illness, prevent unnecessary acute hospital admissions and premature admissions to long-term care, support timely dischargefrom hospital and maximise independent living wherever possible. Multi-professional staff will collaborate with individuals to ensure they achieve their full potential in terms of their level of independence, health and wellbeing.

30 hours per week, permanent, Monday - Friday

We are looking for asenior social worker to join our team who is committed to providing astrengths based approach and passionate about supporting patients to achieve/regain independence.

As a senior social workerwithin The Community Inpatient Unit you would be expected to undertake needsassessments in line with The Care Act (2014), complete Mental Capacity/ Best Interest assessments, design and arrange support packages to facilitate safeand timely discharges and work closely with our nursing and therapy teamsamongst other social work functions. .

Relevant Post Qualifying training undertaken

Relevant registration with professional body

Experience of working with adults.

Working knowledge of relevant legislation
