Een geestelijke gezondheidszorgaanbieder in Amsterdam zoekt een ambulant verpleegkundige/begeleider om deel uit te maken van het FACT-team Ouderen. Je biedt herstelgerichte zorg aan ouderen met ernstige psychiatrische aandoeningen. Je werkt nauw samen met andere zorgprofessionals en neemt regie over de behandeling van cliënten in hun eigen omgeving. De ideale kandidaat heeft een BIG-registratie en kennis van sociale pedagogische hulpverlening of social work.

Een rijbewijs is vereist voor deze rol.
