We are seeking a dynamic, highly motivated Intensive Case Management/Housing Worker, who demonstrates our values of compassion, accountability, responsiveness and empowerment; to our clients, families and each other.
Reporting to the Manager of Community Support Services (CSS) and the Manager for Housing Services, this position provides intake coordination, intensive case management, and housing support to asylum seekers and refugee claimants participating in the Asylum Seeker Assistance Program (ASAP). The role is responsible for conducting assessments, coordinating admissions, delivering mobile and on‑site supports, and assisting clients to stabilize and transition from transitional housing to permanent housing in the community.
Services are delivered in collaboration with Wesley Halton Housing and community partners, using a client‑centered, trauma‑informed approach that prioritizes safety, dignity, and individual needs.
The Asylum Seeker Assistance Program (ASAP) program is to be piloted by a Halton Region–funded project, delivered by Wesley that supports asylum seekers and refugee claimants through coordinated intake, transitional housing, and intensive case management. The program helps participants stabilize, access essential community, settlement, health, and income supports, navigate immigration requirements, and secure permanent housing in the community of their choice.
This position is a unionized, Full‑time, temporary position. The rate of pay is $26.88 per hour. The hours for this position are 40 hours per week.
Key Responsibilities- Provide intensive, individualized case management support to asylum seekers and refugee claimants residing in Wesley‑operated transitional housing, with the goal of stabilization, independence, and transition to permanent housing.
- Conduct comprehensive intake and needs assessments for clients being referred into the transitional housing program, including coordination with Halton Region’s shelter intake system and/or Peel Region’s Reception Centre to ensure smooth and timely transfers.
- Deliver mobile and on‑site case management services addressing housing stability, income supports, health and mental health needs, settlement services, and other resettlement priorities.
- Connect clients to essential community, regional, provincial, and federal services, including Ontario Works, healthcare providers, Legal Aid, mental health supports, food security programs, IRCC‑related services, and other settlement resources.
- Support clients in navigating immigration‑related processes, including booking and attending required IRCC appointments such as the Refugee Medical Exam (RME), and providing orientation to Canada’s settlement and service systems.
- Provide intensive housing assistance, including housing planning, landlord engagement, referrals, and support with applications, to help clients secure and transition into permanent housing in their community of choice.
- Collaborate closely with the Program Housing Worker and Wesley’s Housing & Homelessness team to ensure coordinated admissions, client flow, and continuity of care within the transitional housing program.
- Coordinate interpreter and cultural supports as required, ensuring services are trauma‑informed, client‑centered, and responsive to language, age, health, and cultural needs.
- Maintain accurate case notes, reports, and data related to client progress, outcomes, and challenges, contributing to program reporting and continuous improvement.
