District Nurse Deputy Leader

In this role, you will be an integral part of our Integrated Care Services, working closely with patients, families, and a multi-disciplinary team to deliver high-quality, person-centred care in the community.

• Work closely with the multidisciplinary team to deliver holistic care, ensuring seamless service delivery for patients and communities.
• Partner with patients and their families to create personalised health plans that address their unique needs and goals, ensuring their voice is central to the care process.
• Actively engage in service improvement initiatives, helping to shape and enhance the care we provide to meet the evolving needs of our patients.
• Support the Team Leader in the smooth running of the service and step into the Team Lead role when necessary, ensuring continuity of care and leadership.
li>Work alongside other healthcare providers to ensure the efficient and effective delivery of services, fostering collaborative relationships across health and social care.

• A registered nurse with experience in community healthcare and a strong understanding of district nursing.
• A compassionate, proactive individual with excellent communication skills, able to build meaningful relationships with patients, families, and colleagues.
