Are you a compassionate, dynamic nurse with a passion for community healthcare? We're looking for an enthusiastic District Nurse Deputy Leader to join our collaborative and supportive team in Harrow. In this role, you will be an integral part of our Integrated Care Services, working closely with patients, families, and a multi-disciplinary team to deliver high-quality, person-centred care in the community.

Responsibilities
  • Work closely with the multidisciplinary team to deliver holistic care, ensuring seamless service delivery for patients and communities.
  • Partner with patients and their families to create personalised health plans that address their unique needs and goals, ensuring their voice is central to the care process.
  • Actively engage in service improvement initiatives, helping to shape and enhance the care we provide to meet the evolving needs of our patients.
  • Support the Team Leader in the smooth running of the service and step into the Team Lead role when necessary, ensuring continuity of care and leadership.
  • li>Work alongside other healthcare providers to ensure the efficient and effective delivery of services, fostering collaborative relationships across health and social care.

Qualifications
  • A registered nurse with experience in community healthcare and a strong understanding of district nursing.
  • A compassionate, proactive individual with excellent communication skills, able to build meaningful relationships with patients, families, and colleagues.
  • A motivated leader who is passionate about developing others, with a commitment to continuous personal and professional growth.
