About the role

The Homefirst Team is part of the integrated community team in West Cornwall, supporting intermediate care needs of older adults. As a Home First Nurse Assessor you will develop assessment, care planning and reablement skills within a dynamic, multidisciplinary team.

We support flexible working arrangements, including part‑time, job share and other flexible patterns. A valid UK driving licence and access to a vehicle for work use are essential.

Key responsibilities
  • Perform assessments, plan and implement care for adults in homes or care facilities.
  • Interpret clinical information, identify problems and set realistic goals.
  • Triage, assess and provide evidence‑based treatment plans.
  • Liaise with community, acute hospitals, adult social care and voluntary services.
  • Work within a lone working environment and manage associated risks.
Qualifications & Experience
  • Degree or diploma in Nursing.
  • NMC registration.
  • Substantial post‑registration clinical experience in a similar community role.
  • Extensive experience working within a multidisciplinary team.
  • Experience managing lone‑working risks.
  • Leadership in complex situations.
  • Awareness of safeguarding policies and procedures.
  • Clear understanding of clinical effectiveness and evidence‑based care.
