The Homefirst Team is part of the integrated community team in West Cornwall, supporting intermediate care needs of older adults. As a Home First Nurse Assessor you will develop assessment, care planning and reablement skills within a dynamic, multidisciplinary team.
We support flexible working arrangements, including part‑time, job share and other flexible patterns. A valid UK driving licence and access to a vehicle for work use are essential.
Key responsibilities- Perform assessments, plan and implement care for adults in homes or care facilities.
- Interpret clinical information, identify problems and set realistic goals.
- Triage, assess and provide evidence‑based treatment plans.
- Liaise with community, acute hospitals, adult social care and voluntary services.
- Work within a lone working environment and manage associated risks.
- Degree or diploma in Nursing.
- NMC registration.
- Substantial post‑registration clinical experience in a similar community role.
- Extensive experience working within a multidisciplinary team.
- Experience managing lone‑working risks.
- Leadership in complex situations.
- Awareness of safeguarding policies and procedures.
- Clear understanding of clinical effectiveness and evidence‑based care.
- Teaching, coaching, mentoring skills for staff and students.
