Community Wellbeing Social Worker (Adult Social Care)
Contract: Permanent Location: Hereford City, Ross on Wye and Leominster (choose your preference)
Hours: 37 hours per week Salary: £36,363 to £40,777 + £3,000 welcome payment (see guidance)

- Conduct Care Act Assessments and person-centred support planning.
- Carry out Mental Capacity Assessments and Section 42 Enquiries.
- Work with diverse adult populations, including those with physical and learning disabilities, mental health needs, and older adults.
- Apply a strength‑based approach to empower individuals and promote independence.
- Engage in crucial CHC and s117 work to ensure seamless support.
- Collaborate with a range of partner agencies to provide holistic care.
- Participate in regular supervision and pursue specialist training to develop into a Best Interest Assessor or Approved Mental Health Professional.
- Registered Social Worker with Social Work England and a qualifying degree.
- Up‑to‑date knowledge of current legislation and guidance.
- Track record of effective, person‑centred assessment and care management.
- Drive to pursue further qualifications such as Best Interest Assessor, Practice Education, or Approved Mental Health Professional.
- Full, clean driving licence and willingness to work across the county.
