Job Description

The Case Manager role supports our clients to ensure high‑quality, consumer‑directed care. The role requires proactive case management and customer service across various funding streams.

Responsibilities
  • Provide information and guidance on services to our clients
  • Complete intake visits and create care plans
  • Conduct initial and ongoing assessments that meet client needs, goals and preferences
  • Ensure services are delivered with dignity and respect in a safe environment
  • Respond and communicate with clients about changing care needs
  • Facilitate support services from external providers
  • Act as an ambassador for the organisation through attendance at community events
  • Maintain and ensure currency of electronic records and case notes
Desired Skills And Experience
  • Proven experience in case management in NDIS or aged care
  • Intermediate skills in Word, Excel and Microsoft Office
  • Experience working with service‑related databases;
    Alayacare experience desirable
  • Current driver's licence, own registered and fully insured vehicle
  • Satisfactory National Police Check
  • NDIS Worker Screening check
  • Passion and commitment to delivering exceptional consumer‑directed care
  • Ability to work autonomously with a supportive and cohesive team
  • Reliability and commitment to promoting organisational growth
  • Ability to prioritise, manage time and deliver services within agreed timelines
  • Good oral and written communication skills
  • Highly motivated, organised, proactive and efficient
