Health Care Support Worker

Overview

Health Care Support Worker role at Locala Health & Wellbeing. Permanent position, part-time with hours negotiable up to 30 hours per week.

• Work with the specialist nurses in the Locala Community Heart Failure Team in Huddersfield to deliver holistic care to patients with heart failure.
• Work in a variety of clinic and community settings, taking blood, performing observations and other clinical tasks to support patient optimisation.
• Support the nurses in delivering care, contact patients, arrange clinics and home visits, and organise your own list of visits and delegated tasks.
• Demonstrate established skills in phlebotomy and patient observations; work as part of a team and also independently in different settings.

The post holder will be required to work with the specialist nurses; service operates Monday to Friday, 8am to 5pm. This post is part-time with hours negotiable up to 30 hours per week. Salary:
Band 3 – £24,937–£26,598.
