• Independently assess, treat, and plan the discharge of patients within the community rehab team.
• Use recognised outcome measures and your clinical reasoning skills to guide effective treatment plans.
• Confidently manage and prioritise your own caseload and comprehensive treatment plans.
• Collaborate closely with other healthcare professionals to ensure comprehensive patient care.
• Engage in continuous learning and improvement to enhance patient outcomes.

• Hold a degree or equivalent qualification in Physiotherapy.
• Recent experience in specialised physiotherapy within the UK.
• Registration with the HCPC as a physiotherapist qualified to work in the UK.
• Experience in an NHS setting is advantageous but not essential.
• Preferably have experience working within a community rehab setting.
• Must be a car driver to facilitate effective community visits.
