This role is integral in supporting timely and effective discharges while reducing readmission rates. You will work closely with individuals at high risk of relapse, ensuring continuity of care from hospital to community settings. • Collaborate with multidisciplinary partners including voluntary agencies, housing services, and substance misuse providers
• Engage effectively with families, carers, and community networks to encourage positive outcomes
• Uphold safeguarding responsibilities, promoting the safety and wellbeing of adults at risk
• Ensure appropriate access to local services and coordinated ongoing care
