Job Summary

We are seeking an experienced and motivated Nurse Practitioner / Advanced Nurse Practitioner to join our friendly, forward‑thinking clinical team for 4‑6 sessions per week. This is an excellent opportunity for a skilled clinician who is passionate about delivering high‑quality, patient‑centred care and working autonomously within a supportive environment.

Responsibilities
  • Clinical Practice and Supervision
    The Nurse Practitioner will work under the direct clinical guidance and supervision of a named General Practitioner, with practice limitations and supervision requirements established via a formal clinical governance framework.
    Assess, plan, implement, and evaluate treatment, interventions, and care for patients presenting with differentiated and pre‑triaged conditions that fall under established GP‑approved protocols (e.g., minor ailments, chronic disease review follow‑ups, specific acute presentations).
  • Prescribing and Management
    Prescribe and review medication for effectiveness, appropriate to patient needs, and in accordance with evidence‑based practice and national and practice protocols, within the NP's experience and the scope approved by the supervising GP.
    Manage both acute and chronic conditions, utilising both drug‑ and non‑drug‑based management, as delegated by the clinical team.
  • Patient Safety and Referral
    Advise patients on general health care and minor ailments.
    Ensure own clinical practice is safe and effective and remains strictly within the boundaries of competence.
    Acknowledge limitations immediately, arranging prompt referral to the supervising GP or other members of the primary and secondary health care team if the presenting condition is complex, undifferentiated, or outside the agreed scope of practice.
  • Quality and Compliance
    In consultations, perform tasks highlighted by Systm One in relation to patients' chronic health conditions, including QOF indicators and enhanced services.
  • Integration with Nursing Team
    Work with the nursing team lead to develop nursing services responding to the changing requirements in general practice.
    Contribute to the development of local guidelines, protocols and standards.
    Attend practice nurse meetings.
  • Administration and Professional Responsibilities
    Record accurate and contemporaneous notes of all consultations and treatments.
    Complete all necessary documentation associated with patient care and registration.
    Review and process data using Read codes to ensure accurate information retrieval for monitoring and audit processes.
    Perform and participate in regular and ad‑hoc audits to ensure high quality of patient care.
    Attend and participate in practice meetings as required (including clinical meetings, TARGET, whole practice meetings and multidisciplinary meetings).
    Recognise and work within own competence and professional code of conduct as regulated by the NMC.
    Highlight any significant events, both clinical and administrative, and attend review meetings.
  • Training and Personal Development
    Full attendance and commitment to learning.
    Engage with contents of educational contract within the practice environment.
    Develop and maintain a Personal Learning Plan.
    Engage with regular appraisals.
    Participate in the education, training and induction of colleagues.
  • Liaison
    Work closely with reception, office and dispensary staff to ensure the smooth running of the practice.
    Work closely with the GPs, one of whom will always be nominated as the dedicated clinical supervisor and advisor.
    Maintain good liaison with other surgeries and agencies including secondary care.
    Be an effective and responsible team member, supporting others and exploring mechanisms to develop new ways of working.
  • Confidentiality
    Respect patient confidentiality and handle sensitive information appropriately.
    Access to confidential information must remain strictly confidential and only be divulged to authorised persons in accordance with policy.
