Chronic Disease Nurse - Dr McKenna & Partners

(band 6 – Salary dependant on experience)

Part time - hours negotiable

We are looking for an experienced Chronic Disease Nurse with good interpersonal and communication skills to join our small, friendly team. Dr McKenna & Partners is a 3 GP Practice with a team of 2 ANP’s, 1 associate Nurse Partner, 1 NP, 2 PN’S, 1 HCA and 7 administration staff. We also have Pharmacists, a Mental Health Nurse and Care Home ANP’s that support our team.

You will possess a wide range of knowledge, as well as being experienced in all chronic disease management areas, particularly respiratory. Previous experience working in General Practice or the community is desired.

The post offers 20 hours per week (although this is negotiable and more or less hours will be considered) with some flexibility as to how this is structured.
