Main duties of the job

In this rewarding role, your core focus will be on delivering direct, face-to-face support to young people. You will be the trusted professional conducting initial assessments, building therapeutic relationships, and guiding them through their care journey. A typical week will see you balancing a varied caseload, from providing one-to-one support to collaborating with families and partner agencies. You'll be a key player in our duty system, using your clinical judgement to triage new referrals and prioritise needs.

1. Job Purpose

To provide a high-quality, specialist mental health service to children, adolescents, and their families within the Fylde and Wyre area. The post holder will act as a named practitioner, undertaking autonomous assessments, formulating care plans, and delivering evidence-based interventions. A key function of the role is to manage clinical risk effectively and contribute to the teams duty and triage system.

Key Responsibilities

Clinical Practice

  • To undertake comprehensive, autonomous assessments of children and young people presenting with a wide range of mental health needs.
  • To formulate, implement, and evaluate care plans in collaboration with young people and their families.
  • To lead on the planning and coordination of safe and effective hospital discharge processes.
  • To provide face-to-face support and interventions, managing an individual caseload with professionalism and compassion.
  • To signpost young people and families to appropriate services and community resources.

Risk Management and Safeguarding

  • To clinically assess and manage risk, including the development, implementation, and ongoing review of dynamic risk management plans.
  • To act as a safeguarding lead for the service, making timely and appropriate referrals to children's social care in line with organisational policies.
  • To possess significant experience in CAMHS and/or childrens child protection work.

Professional and Team Responsibilities

  • To participate actively in the teams duty rota, triaging new referrals and prioritising workload effectively.
  • To work collaboratively within the multi-disciplinary team (MDT), providing support to colleagues and contributing to a positive team culture.
  • To work in accordance with all local policies, procedures, and clinical thresholds.
  • To maintain accurate, timely, and confidential clinical records in line with professional and organisational standards.
