We are looking for a qualified Nursing associate who would be willing to upskill to assist with our practice nurses in managing our patients with chronic diseases. This would include upskilling to be able to manage chronic kidney disease, hypertension and respiratory related diseases.
We are a large forward thinking practice and we are willing to invest in the right applicant.
Main duties of the jobThe post holder is a Nursing Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi‑disciplinary team within General Practice.
As a Nursing Associate, you will work under the supervision of and in partnership with the Nursing team to provide and deliver a high standard of nursing care. You will contribute to the on‑going assessment, providing and monitoring care based on evidence and working in partnership with the multi‑disciplinary team.

Job responsibilities- Engage in positive relationships with patients, carers, families and other staff based on respect and within appropriate boundaries.
- Act in a professional capacity at all times.
- Set therapeutic limits with patients, carers or relatives.
- Communicate with the patient about care planning, risk assessment needs.
- Communicate complex and sensitive information verbally and in writing.
- Actively contribute during handovers to meet the care needs of patients.
- Actively contribute to staff meetings.
- Confidently and accurately convene patient information in the role of an associate nurse for MDT meetings.
- Formulate care plans under the supervision of registered staff.
- Communicate risk issues as they arise to ensure safe care.
- Communicate concerns relating to patient behaviour or mental state to other members of the multidisciplinary team.
- Demonstrate evidence‑based knowledge in the main physical health problems that adult patients may present with: high blood pressure, diabetes, obesity, epilepsy, respiratory issues, constipation, lack of personal hygiene, including foot care.
- Undertake baseline physical observations and record results correctly, and identify anomalies for referral to a first level nurse or medical staff.

- Understands and acts in line with NMC professional standards for practice contained within the code.
- Understand the requirements for NMC professional revalidation.
- Experience of working in teams under appropriate supervision.
- Insight into how to evaluate own strengths and development needs.
- Venepuncture trained.
- Effective time management (Planning & Organising).
- Ability to work as a team member and autonomously.
- Good interpersonal skills.
- Ability to follow clinical policy and procedure.
- ECGs.
- Competent in the use of Office and Outlook.
- Registered Nursing Associate on the NMC register.
- GCSE Grade A‑C in Maths and English or Skills Level 2 Maths and English or equivalent.
- Experience of working with the general public.
- Experience of working in a healthcare setting.
- Experience of working in a primary care environment.
