Qualifications- A heart to serve patients and families with a passion for high‑quality care.
- Education: Graduate of an accredited masters, post‑masters, or doctoral nursing program with preparation as a Nurse Practitioner.
- Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner and full DEA prescriptive authority. Must maintain national certification in the Nurse Practitioner specialty.
- Experience: 2 years in chronic disease management/long‑term care within the past 5 years; hospice and/or palliative background preferred.
- Required: Reliable transportation; able to sit, stand, bend, move intermittently, and lift 80‑100 lbs.
